Human error is blamed for a diesel fuel spill into the Ohio River.
Coast Guard and EPA officials say someone left a valve open, yesterday, at a Duke Energy facility, dumping 5,000 gallons of fuel into the water.
Cincinnati Water Works officials say the drinking water is safe.
